Create an email data destination with the AT&T Mobile Forms web portal

1.       Hover over the “Manage Forms” drop-down arrow Image of the drop-down arrow.  and click Data Destinations.

2.       Click the Create Data Destination button.

3.       Click Email.

4.       In the “Name” field, name the data destination.

5.       To auto-link new forms to this destination, check the checkbox to the left of “Auto-Link new forms to this destination”.

6.       Under SMTP Server, select the server settings by clicking the radio button to the left of the desired option.

7.       Click the Filtering tab.

8.       Use the checkboxes under “Data Record Filters” to configure the filtering rules.

9.       Click the Email Headers tab.

10.    Under “Reply-to Email Address”, click the radio button to the left of the desired option.

11.    Under “Email Subject Line Expression” enter the desired subject line expression.

12.    Under “Distribution Type” click the radio button to the left of the desired option.

13.    Check the checkbox to the left of “Send email to Data Record Submitter” to enable this option.

14.    Check the checkbox to the left of “Send email to Data Record Submitter’s alternate email address” to enable this option.

15.    Check the checkbox to the left of “Send email to the users with ‘Data Record Submission Listener’ checked in the Data Record Submitter’s group” to enable this option.

16.    Under “Groups to Include” select from the available groups under “Unselected Groups” and then click the right-pointing arrows Image of the right pointing arrows..

17.    Under “Other Addresses to Include” enter a list of email addresses desired.

18.    Under “Form Labels to Extract” enter a list of labels desired.

19.    Scroll back up to the top of the screen and then click the Email Body tab.

20.    Under “Email Contents” click the radio button to the left of the desired option.

21.    Click the checkbox to the left of “Include a hyperlink to the form submission in the portal at the top of the email” to enable this option.

22.    Click the Attachments tab.

23.    Click the checkbox to enable attachments.

24.    Click the Create button.
