Create an email data destination with the AT&T Mobile Forms web portal

  1. Hover over the drop-down arrow Image of the drop-down arrow. to the right of “Manage Forms”.

  2. Click on Data Destinations in the menu.

  3. Click the Create Data Destination button.

  4. Click Email.

  5. Click in the field under “* Name” and enter a name for the data destination.
  6. To auto-link new forms to this destination, check the checkbox to the left of “Auto-Link new forms to this destination”.
  7. Under SMTP Server, select the server settings by clicking the radio button to the left of the desired option.
  8. Click the Filtering tab.

  9. Use the check-boxes under “Data Record Filters” to configure the filtering rules.
  10. Click the Email Headers tab.

  11. Click the radio button to the left of “Reply-to Email Address” to set up a custom reply-to email address.
  12. Click in the field under “Email Subject Line Expression” to customize he subject line for the emails.
  13. Under “Distribution Type” click the radio button to the left of the desired options.
  14. Check the checkbox to the left of “Send email to Data Record Submitter” to copy the form submitter.
  15. Check the checkbox to the left of “Send email to Data Record Submitter’s alternate email address” to send an email to the submitter’s alternate email address.
  16. Check the checkbox to the left of “Send email to the users with ‘Data Record Submission Listener’ checked in the Data Record Submitter’s group” to copy users following the form in emails.
  17. Scroll down the page.

  18. Click the name of the desired group(s) Under “Unselected Groups” and then click the right-pointing arrows Image of the right arrows..
  19. Click in the field under “Address List” to manually enter email addresses that should be carbon copied in email messages.
  20. Click in the field under “Question Labels” to enter a list of desired labels.
  21. Click the Email Body tab.

  22. Under “Email Contents” click the radio button to the left of the desired option.
  23. Click the checkbox to the left of “Include a hyperlink to the form submission in the portal at the top of the email” to enable this option.
  24. Click the Attachments tab.

  25. Click the checkbox to the left of “Send photos, sketches, audio files, and similar form data as separate file attachments to the message. Note that selecting this option does not affect the use of these files in documents like PDF, DOCX, etc.” to enable attachments.
  26. Click the Create button.
