Create a form in the Xora StreetSmart web application

  1. Access the website at http://streetsmart.xora.com.
  2. In the box below “Username:”, enter your username.
  3. In the box below “Password:”, enter your password.
  4. In the box below “Company ID:”, enter your company’s ID.
  5. Click Login.

  1. Hover over “Forms”, and then click Forms.

  1. Click Create Form.

  1. Click in the field next to “Name:” and enter a name for your form.
  2. Click in the field next to “Display Name:” and enter the desired display name.
  3. Click in the field next to “Description:” and enter a description of the form.

  1. Click Next.

  1. Click New Field.

  1. Click in the field next to “Display Name:” and enter a name for the new field.
  2. Click in the field next to “Field Ref:” to edit the reference name for the new field.
    Note: This field will auto-populate to the name entered in the previous step.

  1. Click the dropdown arrow to the right of “Field Type:” and select the type of field that you would like to create.

  1. Click in the field next to “Field Length:” and enter the length of the new field.
    Note: This number will vary depending on the type of field selected.

  1. Click the checkbox to the right of “Required:” to make the information requested in the field mandatory for the worker.
  2. Click in the field next to “Default value:” and enter the desired default value.
  3. Click Apply.

  1. The new field saves.
    Note: To create more fields, click New Field and repeat the steps above.
  1. Click Next.

  1. Click the checkbox next to “Active form:” to enable the form to be viewed on the workers mobile device.
  2. Click the checkbox next to “Available on Forms Menu” to allow workers to see the form by navigating to the Forms options in the mobile application.

  1. Click the checkboxes in the “Select the Groups this form will be made available to” box to select the groups that the form will be assigned to.

  1. Click Next.

  1. Select the desired email notification options.
  2. Click Save & Close.

  1. The new form displays.
