Add a custom form to a shared contact using the pdvConnect web interface

  1. Click Account Setup, and then click Shared Contacts.

  2. Click the edit icon next to the shared contact you want to edit.

  3. Under Shared Contact Permissions, use the radio button to select the desired custom forms, and then click Save.
    Note: If no forms are available when selecting the dropdown, custom forms are created by the pdvConnect Care team. Contact pdvConnect Care at care@pdvConnect.com to schedule an appointment to set up forms.
